Procedural Information for Prospective Applicants and Preliminary Application Form

Dear Applicant,

Thank you for your interest in the graduate program in Computational and Applied Mathematics. If you are interested in attending graduate school at ¹ÏÉñÍø and studying mathematics or statistics, then it is advised to complete both a formal application sent to the  and a preliminary application sent to the Department.

The Office of Admissions uses the formal application to enter the potential graduate student into the University's computer system and to make sure all the University required documents are complete. After screening the candidate for University requirements, the Office of Admissions will forward the application to the Department. International students need to forward a formal application to the .

The preliminary application notifies the department that a potential graduate student is interested in the program, and it is not the official application to the University. It is used internally within the department to expedite the review process. While not mandatory, submission of a preliminary application will allow us to provide advice regarding your candidacy for admission. Please ONLY send the preliminary application (2 pages) to the department. All other documents required for the formal application must be submitted to the . You can print and complete the preliminary application form from the portable document format (PDF) provided below.
