- Assumption of Risk Form (To ensure liability coverage during field activities with student classes and volunteers, we now have a single form to provide risk assumption for the department. This form will need to be signed by all participants and all volunteers that participate in any field or sea activity through the department either as a classroom activity or research assistant of faculty.)
