Our offices currently don't have the resources to provide our transgender student's hormone therapy treatment. In the near future, we hope to be able to provide this service. - Patients scheduling a routine pelvic exam will be asked to fill out a short intake form at the appointment to gather information about health concerns, as well as medical and sexual histories.Ìý
- Be sure to bring your ¹ÏÉñÍø I.D.
Instructions: Check in for your appointment 10 minutes before your scheduled appointment time and before entering Student Health Services.ÌýBe sure to have your ¹ÏÉñÍø I.D. with you at check-in.

Log into yourÌý.

Check-in using your phone or laptop. Your device location must be turned on for check-in.ÌýNote: Appointment Self-check-in is only enabled within a certain parameter of the clinic's location

Go to the Home page and scroll down to see your appointment information.

If you see a button called "Forms", select and complete the form(s) presented.ÌýNote: Not all appointment reasons require this.

Then select the "Check-in" button when it appears.

Once you are checked in come into the office and let the front office staff know you are here for your appointment, and you have checked in.
